I've done every ascendant challenge for the past six weeks. I always make sure to collect the chest and leave through the portal (I took the sword through with me in Honed Edges). Go to the white orb where the relic sword spawned and when you get near it it'll open the portal. Haul ass to the exit portal but be careful. Some of the rocks you jump on disappear also there are the big rocks flying through the air that can knock you off.
